Build feedback forms

Create a feedback form on a playtest, pick question types, share the link or QR code, and manage active forms.

A feedback form is a short questionnaire your playtesters fill in on their own phone or laptop, with no account. This page shows how to build one on a playtest, what question types you have, and how to share and manage the form.

What this page helps you do

  • Create a feedback form on a playtest.
  • Choose the right question type for each question.
  • Share the form as a link or a QR code.
  • Turn a form off, edit it, or delete it.

You'll start from a playtest's Feedback Form tab (sidebar Playtests, click the game, open a playtest).

Before you start

  • Log the playtest first (see Log a playtest session). Forms belong to a playtest.
  • Everyone on the team who can open the playtest can create and edit forms.

Open the Feedback Form tab

On the playtest page, click the Feedback Form tab. The heading reads "Feedback Form" with the line "Create and manage feedback forms for external testers", and a Create Form button at the right.

Helpful details

  • Key areas: the list of this playtest's forms, each on a card. With no forms yet the card says "No feedback forms yet".
  • What you can do: a playtest can have several forms, each with its own link. Use that to ask different groups different questions.
  • Good to know: the labels on this tab (Create Form, View Responses, Copy Link, Open, Edit, Add Question) are in English in every language.

Create a form

Click Create Form. A "Create a new feedback form" card opens with the form's title and description, the question list, and Add Question.

Helpful details

  • Key areas: Form Title (starts as "Feedback Form"; the playtester sees it at the top of the page), Description (Optional), then the questions. Every new form starts with two questions, Your Name and Your Email, both required and marked Protected.
  • What you can do: click Add Question to open the "Add Question" dialog. Drag the handle at the left of a question card to reorder. Click the gear on a card to edit it, the trash can to delete it. Click Create Form at the bottom to save.
  • Good to know: Your Name and Your Email cannot be deleted or edited ("The name and email fields cannot be deleted"). A form needs at least one question to save.

Pick a question type

In the "Add Question" dialog, choose a Question Type, write the Question Title, optionally a Description (optional) shown above the question, and turn Required question on or off. Click Save Question.

Helpful details

  • Key areas: the types are:
    • Short Text: one line. "Where did you play?"
    • Long Text/Comment: a paragraph. "What confused you?"
    • Rating: a scale. Pick a Rating Type (Auto, Stars, Smiley Faces, Hearts or Numbers), a Minimum Rating and Maximum Rating (1 to 10), and a Minimum Description and Maximum Description such as "Not engaged at all" and "Very engaged".
    • Yes/No: a single switch. "Would you play again?"
    • Dropdown: pick one option from a list.
    • Multiple Choice: pick one option, shown as a set of buttons.
    • Checkboxes: pick several options.
    • HTML Content: a block of text or an image for the playtester to read, not a question. You can use tags like p, img and strong.
    • Separator: a line between groups of questions.
  • What you can do: Dropdown, Multiple Choice and Checkboxes need at least one option: type it in "Add new option..." and press Enter or the plus button.
  • Good to know: "Multiple Choice" means one answer; Checkboxes is the type for several answers. There is no date or file-upload question.

What to ask

Keep the form short: five to eight questions after the name and email. Ask about the goal you wrote in Playtest Goal, plus a few questions that work for every session.

Helpful details

  • Key areas: a dependable core set:
    1. Did the game end at the right time? (Multiple Choice: Too short / Right length / Too long)
    2. What was your favorite moment? (Long Text/Comment)
    3. What confused you? (Long Text/Comment)
    4. Would you play this again? (Yes/No)
    5. How much fun did you have? (Rating 1 to 7, to match your own engagement scale)
  • What you can do: add one or two questions about this session's hypothesis ("Did the new scoring rule feel fair?"), then stop.
  • Good to know: put the easy questions first and the open "what would you change?" question last. Leave most questions optional; the name and email are already required, so only make a question required when a blank answer would make the whole response useless.

Share the form

Each form card shows a QR code and a row of buttons. Share the link where your playtesters are: a Discord channel, an email, a slide at the end of a convention demo.

Helpful details

  • Key areas: the QR code (click it to download a PNG), View Responses, Copy Link, Open, Deactivate / Activate, Edit and a red delete button.
  • What you can do: Copy Link copies the public address (/feedback-form/<short id>). Open opens it in a new tab so you can test it yourself.
  • Good to know: playtesters do not need a Boardssey account. Each browser can submit a form once; a second visit shows "Already Submitted". If you test the form yourself, use a private window so you do not use up your own browser's submission.

Edit, pause or delete a form

Use the buttons on the form card.

Helpful details

  • Key areas: the Active / Inactive badge next to the title.
  • What you can do:
    • Deactivate closes the form: the link shows "This form is no longer active". Activate reopens it. Responses are kept either way.
    • Edit opens the builder on the card. Change the title, description or questions, then click Update Form.
    • The red trash button deletes the form after you type CONFIRM. Its responses are deleted with it.
  • Good to know: editing while responses exist is allowed. Old responses keep their old answers; the responses view labels each answer with the current question title, so avoid changing a question's meaning after people have answered it. Add a new question instead.

Tips & common questions

Can I include images in a form? You can show an image with an HTML Content block (an img tag). Playtesters cannot upload images or files.

Can a playtester save and finish later? No. The public form has no draft saving. If they close the tab they start over, which is one more reason to keep the form short.

Are responses anonymous? Not by default. Your Name and Your Email are required on every form and cannot be removed. Anyone who submits also gets added to the game's Playtesters tab.

Can I reuse a form on another playtest? Not today. Each form belongs to one playtest, and there is no copy button. Keep your question list in a note and rebuild it; it takes a couple of minutes.
