Welcome to the Feedback Form tool! This powerful feature lets you create custom questionnaires to collect structured feedback from your playtesters. Whether you're running a closed beta, gathering user testing data, or collecting post-game feedback, this tool makes it easy to design forms and analyze responses.

What is the Feedback Form Tool?

The Feedback Form tool allows you to:

  • Create custom questionnaires with various question types

  • Share forms easily via links or QR codes

  • Collect responses from external testers (no login required)

  • View and manage all feedback in one place

  • Activate or deactivate forms as needed

Perfect for playtesting sessions, user research, and gathering structured feedback on your games!

Getting Started

Step 1: Access the Feedback Form Tool

  1. Navigate to your playtest: Playtests → [Your Game] → [Playtest]

  2. Click on the Feedback Form tab

  3. You'll see the feedback form management interface

Step 2: Create Your First Form

  1. Click the "Create Form" button in the top right

  2. Enter a Form Title (e.g., "Beta Test Feedback")

  3. Optionally add a Description (e.g., "Help us improve our game!")

  4. Start adding questions (see below)

  5. Click "Create Form" when ready

Note: Every form automatically includes "Your Name" and "Your Email" fields. These cannot be deleted and are always required.

Understanding Question Types

The tool offers 9 different question types to help you collect the right kind of feedback:

1. Short Text

Best for: Names, short answers, single-word responses

  • Single-line text input

  • Perfect for quick answers

  • Example: "What's your favorite color?"

2. Long Text/Comment

Best for: Detailed feedback, explanations, open-ended responses

  • Multi-line text area

  • Great for detailed feedback

  • Example: "What did you like most about the game?"

3. Rating

Best for: Numeric ratings, satisfaction scales, engagement levels

  • Visual rating interface

  • Rating Types:

    • Auto (Default) - Automatically chooses best display

    • Stars ⭐⭐⭐⭐⭐

    • Smiley Faces 😊

    • Hearts ❤️

    • Numbers 1-5

  • Customizable range: Set minimum and maximum values (1-10)

  • Optional labels: Add descriptions for min/max (e.g., "Not engaged" to "Very engaged")

  • Example: "Rate your overall experience (1-5 stars)"

4. Yes/No

Best for: Binary questions, simple choices

  • Radio buttons with Yes/No options

  • Quick decision questions

  • Example: "Would you recommend this game to a friend?"

5. Dropdown

Best for: Single choice from many options

  • Dropdown menu with multiple options

  • Saves space when you have many choices

  • Example: "Which game mode did you prefer?" (with options like Campaign, Multiplayer, Co-op)

6. Multiple Choice

Best for: Single choice from a list

  • Radio buttons showing all options

  • All choices visible at once

  • Example: "What device did you play on?" (Mobile, Tablet, Desktop)

7. Checkboxes

Best for: Multiple selections allowed

  • Checkboxes for selecting multiple options

  • Users can choose all that apply

  • Example: "Which features did you enjoy?" (Story, Graphics, Music, Multiplayer)

8. HTML Content

Best for: Rich text, images, formatted content

  • Add HTML content to your form

  • Use for instructions, images, or formatted text

  • Supports HTML tags like <p>, <img>, <strong>, etc.

  • Example: Add game screenshots or formatted instructions

9. Separator

Best for: Visual organization, grouping questions

  • Visual divider between questions

  • Helps organize long forms into sections

  • No input required from users

Building Your Form

Adding Questions

  1. Click "Add Question" button

  2. Select the question type from the dropdown

  3. Enter your Question Title (required)

  4. Optionally add a Description (appears before the question)

  5. Toggle "Required question" if users must answer

  6. Configure type-specific options (see below)

  7. Click "Save Question"

Configuring Question-Specific Options

For Rating Questions:

  • Choose Rating Type (Stars, Smileys, Hearts, Numbers, or Auto)

  • Set Minimum Rating (default: 1)

  • Set Maximum Rating (default: 5)

  • Optionally add Minimum Description (e.g., "Not engaged")

  • Optionally add Maximum Description (e.g., "Very engaged")

For Dropdown, Multiple Choice, or Checkboxes:

  • Add options one by one

  • Type an option and click the + button

  • Edit options by clicking in the input field

  • Remove options with the trash icon

  • At least one option is required

For HTML Content:

  • Enter your HTML code (e.g., <p>Welcome to our feedback form!</p>)

  • Use any valid HTML tags

  • Perfect for images: <img src="url" alt="description" />

For Separator:

  • No configuration needed

  • Just adds a visual break

Organizing Questions

Drag and Drop Reordering:

  • Click and hold the grip handle on the left of any question

  • Drag it to a new position

  • Release to drop it in place

  • Questions are automatically saved in the new order

Tips for Organization:

  • Start with name and email (automatically included)

  • Group related questions together

  • Use separators to divide sections

  • Put most important questions first

  • End with optional detailed feedback

Editing and Deleting Questions

Edit a Question:

  1. Click the Edit button (gear icon) on any question card

  2. Modify the question settings

  3. Click "Save Question"

Delete a Question:

  1. Click the Delete button (trash icon) on any question card

  2. Confirm deletion

  3. Note: Name and Email fields cannot be deleted

Managing Your Forms

Form Status

Each form has an Active/Inactive status:

  • Active (Green Badge): Form is live and accepting responses

  • Inactive (Red Badge): Form is paused and not accepting responses

To Change Status:

  • Click "Activate" or "Deactivate" button on any form

  • Status changes immediately

Why Deactivate?

  • Temporarily stop accepting new responses

  • Keep forms available for reference

  • Re-activate anytime to resume collection

Editing Forms

  1. Click the "Edit" button (gear icon) on any form

  2. Modify the form title or description

  3. Add, edit, or reorder questions

  4. Click "Update Form" when done

Important: Editing a form doesn't affect existing responses. You can modify questions and save changes anytime.

Deleting Forms

  1. Click the Delete button (red trash icon) on any form

  2. Confirm deletion in the dialog

  3. Warning: This permanently deletes the form and all its responses

Before Deleting:

  • Export or save any important responses

  • Consider deactivating instead if you might need it later

Sharing Your Forms

Getting Your Form Link

  1. Find your form in the list

  2. Click "Copy Link" button

  3. The form URL is copied to your clipboard

  4. Share this link with your testers

Form URLs look like:

No Login Required:

  • Testers can access forms directly via the link

  • No account needed to submit feedback

  • Works on any device

Using QR Codes

Each form automatically generates a QR code:

  1. View QR Code: The QR code appears on the left side of each form card

  2. Download QR Code: Click the QR code image to download a high-resolution PNG

  3. Print and Share: Print QR codes for in-person playtesting sessions

  4. Display: Show QR codes at events, conventions, or testing sessions

QR Code Tips:

  • Print large enough to scan easily

  • Test the QR code before printing

  • Include a short URL below the QR code as backup

  • Use for physical events and printed materials

Opening Forms

Click "Open" to view your form as testers will see it:

  • Opens in a new tab

  • See exactly what testers experience

  • Test the form yourself before sharing

  • Verify all questions work correctly

Viewing Responses

Accessing Responses

  1. Click "View Responses" on any form

  2. The form expands to show all submissions

  3. Responses are listed chronologically (newest first)

Understanding Response Cards

Each response shows:

Header Information:

  • Respondent Name (or "Anonymous" if not provided)

  • Submission Date & Time

  • Email Address (clickable mailto link if provided)

Response Content:

  • All question titles with corresponding answers

  • Long answers displayed in full

  • Short answers shown inline

  • Responses organized clearly

Managing Responses

Reload Responses:

  • Click "Reload" button to refresh the list

  • Check for new submissions

  • Updates in real-time

Email Respondents:

  • Click the email address button on any response

  • Opens your email client with their address

  • Perfect for follow-up questions

Close Responses View:

  • Click "Close" to collapse the responses section

  • Form card returns to normal view

Best Practices

Form Design

Keep It Focused:

  • Limit to 10-15 questions for best response rates

  • Focus on most important feedback areas

  • Remove unnecessary questions

Question Order:

  • Start with easy questions (name, email)

  • Put important questions early

  • End with optional detailed feedback

  • Use separators to organize sections

Question Types:

  • Use Short Text for quick answers

  • Use Long Text for detailed feedback

  • Use Rating for quantitative data

  • Use Multiple Choice for structured choices

  • Use Checkboxes when multiple answers are valid

Collecting Feedback

Before Sharing:

  • Test your form yourself

  • Verify all questions work correctly

  • Check that required fields are marked

  • Ensure form is active

Sharing Strategies:

  • Email: Send form links via email

  • QR Codes: Print for in-person sessions

  • Social Media: Share links on platforms

  • Website: Embed or link from your site

Response Management:

  • Check responses regularly

  • Follow up with testers via email

  • Export data if needed for analysis

  • Keep forms active during active testing periods

Question Writing

Write Clear Questions:

  • Use simple, direct language

  • Avoid jargon or technical terms

  • Be specific about what you want

  • Keep questions concise

Use Descriptions:

  • Add context with question descriptions

  • Explain what kind of feedback you need

  • Provide examples if helpful

Required vs. Optional:

  • Mark essential questions as required

  • Leave detailed feedback optional

  • Balance required questions (too many = abandonment)

Common Use Cases

Post-Playtest Survey

Structure:

  1. Name & Email (automatic)

  2. Rating: "Overall experience" (1-5 stars)

  3. Multiple Choice: "Favorite game mode?"

  4. Checkboxes: "What did you enjoy?"

  5. Long Text: "What would you improve?"

  6. Yes/No: "Would you play again?"

Feature Feedback Form

Structure:

  1. Name & Email (automatic)

  2. Dropdown: "Which feature are you testing?"

  3. Rating: "How easy was it to use?" (1-5)

  4. Long Text: "Describe your experience"

Bug Report Form

Structure:

Troubleshooting

Form not accepting responses:

  • Check that form is Active (green badge)

  • Verify the form link is correct

  • Try opening the form in a new tab

  • Check that required fields are filled

Questions not appearing:

  • Make sure you saved the question after adding

  • Verify form is active

  • Check that you're viewing the correct form

  • Try refreshing the page

Responses not showing:

  • Click "Reload" to refresh responses

  • Verify form is active and receiving submissions

  • Check that you're viewing the correct form

  • Clear browser cache if needed

QR code not working:

  • Ensure QR code is large enough to scan

  • Check lighting conditions (bright, even light)

  • Verify QR code image downloaded correctly

  • Test the link manually to ensure it works

Can't edit questions:

  • Make sure you clicked "Edit" on the form first

  • Verify you have permission to edit

  • Try refreshing the page

  • Check that form isn't being edited elsewhere

Form link not copying:

  • Try clicking "Copy Link" again

  • Check browser clipboard permissions

  • Manually copy the URL from the address bar

  • Verify you're on the correct form

Tips for Success

Creating Effective Forms

Start Simple:

  • Begin with a few essential questions

  • Add more questions as you learn what works

  • Test with a small group first

Iterate and Improve:

  • Review responses to identify missing questions

  • Edit forms based on feedback patterns

  • Deactivate old forms and create improved versions

Use Appropriate Question Types:

  • Match question type to the data you need

  • Use ratings for quantitative feedback

  • Use long text for qualitative insights

  • Use checkboxes for multiple selections

Maximizing Response Rates

Keep Forms Short:

Export Data:

  • Copy response data if needed for analysis

  • Use email addresses for follow-up surveys

  • Track response rates over time

Privacy and Data

What's Collected:

  • Responses to all questions

  • Respondent name and email (if provided)

  • Submission timestamp

  • All form data

Data Storage:

  • Responses stored securely

  • Accessible only to account owners

  • Can be deleted with form deletion

Privacy:

  • Testers don't need accounts

  • Forms are shareable but responses are private

  • Email addresses visible only to form owners
